Necessary Documents to send us:
1. Complete 2008 Registration Form. You may print a registration form or fill out an online registration form. Forms can also be picked up Monday - Friday at Children's Services 2715 Bainbridge Avenue Bronx, NY 10458.
The information provided on the Registration form provides the basis for refreshing the league's data base each year. Therefore, it is essential that all information be current and correct. Note: If a parent elects to use a post office box for a mailing address, then the player's legal street address must also be displayed at some place on the form.
2. Registration Fee. Money Orders only. Players will NOT receive their uniform without having paid their full registration. Player fees are as follows:

3. Copy of the players Birth Certificate. Note. For those players born outside of the United States we will need a copy of the players United States issued Passport or Greencard. Originals of both will be required of players upon entry registration to some tournaments.
4. Copy of the Players latest report card.
5. Two wallet size photos of player.
6. Proof of address such as copy of utility bill, etc.

Registration Policy:
Registration is on a first come, first serve basis. Season begins in April. As soon as uniforms are issued, no refunds will be given.
Eligibility Requirements:
a. Children must live within the League boundaries.
b. Children must have reached the age of five (5) on or before April 30th 2008 and must not have reached the age of 16 prior to May 1st 2008.
c. All children must be in school.
